Honoree Nomination Checklist & Online Form

Your nomination package must contain the following documents upon submission:

  1. Complete this nomination form with contact information for nominee and nominator
  2. Biography of the nominee (500-550 words) that includes professional background, community work, board positions, other information relevant to the award’s criteria (section provided in form below)
  3. Upload two letters of recommendations
  4. Release & Authorization signed by the nominee (Email this link http://connectingfaithandbusiness.com/nominee-agreement-form/ to nominee for authorization)
